Insurance Coverage and Discount

Insurance coverage is only accepted at my Montclair location. 

I accept some forms of insurance via ASHLink.  AshLink works with Aetna Medicare Advantage PPO, LA Care Health Plan, SCAN Health Plan, American Specialty Health Group Inc, and a discounted program ChooseHealthy. 

Please click on the box below to fill out the insurance intake form.  You will provide the following information so I can submit a query for eligibility:

• First and Last Name
• Date of Birth
• Insurance company and ID#

I do require a check of eligibility before your session to verify plan activity, co-pay, and office visit limitations that may apply. 

Once you submit your information on the intake form, please send me a message here to let me know you completed the form. 

***ChooseHealthy Program is a discounted program.  You will pay out of pocket for your services with a discounted price.  In order to receive your discount, you will need to provide a ChooseHealthy Discount Certificate.
